When diet and exercise didn't do the trick, three Dallas moms chose plastic surgery to get back their pre-baby bodies. Now they look better than ever.

Dr. Steven J. White calls is the "postpartum perk-up" or the "mommy makeover."

Tummy tuck. Breast implants and/or a lift. Liposuction around the thighs and hips. All the procedures that get rid of the stuff those sweet little bundles of joy leave behind on their way out.

Local plastic surgeons have seen a significant increase in women having post-baby procedures in the last five years. Breast augmentation was the top surgical cosmetic procedure for women last year for the first time, according to the American Society for Aesthetic Plastic Surgery. Liposuction was second; abdominoplasty (or tummy tuck) was fourth.

But if we work hard enough, can't we get rid of the baby fat? Not necessarily. Some women accept the changes in their bodies as a rite of passage into motherhood. These days, though, more moms are choosing surgery instead.

"It's not a sign of defeat to have a tummy tuck," says Dr. White, who does more of them than anything else at his practice, USA Plastic Surgery in Dallas. "One of my patients weighted 110 and taught Pilates and yoga. Loose skin won't go away. Muscles will only tighten so much."

A friend of White's once used a bag of balloons to describe it. She laid one on the counter, never blown up. She then blew up a balloon and deflated it, laying it on the counter. Then another one, twice, and did the same thing. Each got bigger as it lay in its deflated state. "This is what happened to my tummy after two kids," White remembers the woman telling her husband.

Surgeons generally recommend waiting to have surgery until you've had your last baby and at least three to six months after you're done breast-feeding. Having surgery when your youngest is close to 3 and can get in and out of his car seat makes things much easier. Ideally, you should have tried diet and exercise, but maybe you've hit a plateau, and you're within 10 to 20 pounds of a realistic weight goal. The more fit you are going into surgery, the better your results.

After Shannon Bush's second child was born, her breast looked like golf balls at the end of a pair of tube socks. Not exactly the figure she remembered from her beach days in Mexico.

And that was okay. Really. Vanity wasn't the main reason Bush decided to have a breast lift. She simply wanted her clothes to fit again.

"Nothing fit me at all anymore," the Lakewood stay-at-home mom remembers. "When I wore my old bras, I had all this skin laying in these big, giant cups. I couldn't wear dresses I used to wear. I'd try on 15 things, and nothing looked right because my nipples were at my waist."

Back to Her Old (and Improved) Self

When Beth Hughes was in high school, her mom got a tummy tuck.

"I didn't even really know what it was," Hughes remembers. "I kind of knew what she did, but she never showed me. She described muscle repair more than skin removal. I was fit and in pretty good shape before I had kids, so I didn't think much about it."

Hughes, who is 5-foot-9, weighted 145 pounds before she had kids and got up to 203 with her first pregnancy. The 29-year-old Keller resident - who dedicates most of her time to her 4-year-old son and 2-year-old daughter - joined Weight Watchers, ran, and lifted weights. She got down to 170 between kids, then down to 150 after her second pregnancy.

But her stomach still looked like it had stretched enough to hold a watermelon, then tried to shrink back up. Which it had. Twice.

"It was hard because I am young," she says. "I worked so hard losing the weight, yet I would look around at my friends and everyone ast the gym, and everyone was so tight and toned, especially in the mid-section. I couldn't lose that much more weight. I was shrinking, but my stomach wasn't."

Hughes' mid-section was her main concern, but she also had some sagging and asymmetry with her breasts. And, while she was considering cosmetic surgery, she figured her thighs could use a little work, too.

She met with Dr. White. Her goal: "Just put me back together."

She decided to do a tummy tuck, liposuction on her thighs, and a breast lift. She did it all at once in March, after lining up two weeks of family help with her kids. She trained for the surgery like she would for an athletic event. Her theory was that if she was in good shape going in, her recovery would be easier, which is true, surgeons say.

Before the surgery, Hughes was a size 10-12. Now, although she weighs the same, she's a 6-8. Her waist went from 31.5 inches to 26. Not only did Dr. White remove extra skin from around her waist, but he also tightened the muscles.

"I look better than before I had the kids," she says, pulling out honeymoon pictures as proof. "I feel almost completely normal. I'd kind of gotten to a point when it was time for me again. I don't want to be the martyr mother who turns around when my daughter is 15 and says, 'Oh, I let myself go.' I wanted to get myself back to where I felt like I was on the inside."

How to Get a Flat Tummy

You Could Undergo Tummy Tuck

The abdomen muscles can get stretched to a point where exercise can't help.

The details

Through one incision in the pubic area that varies from a few inches to one that goes from hip to hip (and possibly another around the belly button), your plastic surgeon can tighten slack muscles and eliminate excess skin. Liposuction can be incorporated to address stubborn fat deposits. For women who delivered via cesarean section, the scar is often removed during a tummy tuck and likely replaced with a less obvious one.

What is Optimized Rapid Recovery Breast Augmentation?

Step One

Your Mirror: Friend or Foe?

If you answered the latter, you're not alone. More than 9.27 million cosmetic plastic surgery procedures were performed in the United States in 2004, and those who had them spent more than $8 billion on their procedures. It's a safe bet they didn't get along with their mirror, either.

If you're ready to finally kiss and make up with the mirror, cosmetic surgery might be the answer for you, too. For the first time, cosmetic surgery is more of a badge of honor than something to hide from your friends. A nip here and a tuck there simply means you value your youthful appearance and want to maintain it as long as possible. In short, you want to look as good as you feel.

If your wish list looks something like this - Angelina Jolie's lips; Cameron Diaz's cheekbones; Jennifer Aniston's eyes; Terri Hatcher's waist - you might to put down your paper and pen and discuss the reality of the list with an experienced cosmetic surgeon.

Although the thought of these beautiful features combined seems perfect, it's really not, say local cosmetic surgeons. Genetics play a major role in everyone's appearance and body shape. Taking bits and pieces from other people you admire isn't going to make you look related to them, much less exactly like them. In fact, the results could be anything but flattering. The goal of cosmetic surgery should be to enhance what you do have so that you look like yourself, only better.

Abdominoplasty

Sometimes all the stomach crunches in the world won't get rid of a protruding abdomen. If this sounds familiar, especially after a pregnancy or significant weight loss, the solution may be a tummy tuck, or abdominoplasty. This is a major surgical procedure that removes excess skin and fat from the middle and lower abdomen and tightens the muscles of the abdominal wall to dramatically reduce the appearance of a protruding abdomen.

Traditional tummy tuck techniques may result in an unnatural, operated look, particularly in the upper abdomen and around the umbilicus, or belly button. Dr. Steven J. White, a board-certified plastic surgeon with USA Plastic Surgery, uses several techniques to obtain a more natural-looking umbilicus. These techniques take more time in surgery; however, he believes the final result is worth this extra effort. For most patients, the scar is well hidden beneath the bikini line.

A tummy tuck can provide an immediate and significant improvement," Dr. White says. "Patients are often amazed by the results. Refined techniques combined with careful follow-up allow patients a quick recovery. Women usually drop several dress sizes, and men go down several pant sizes."

Breast Lift

A breast lift, or mastopexy, is a procedure often performed to restore the youthful appearance to the breast after pregnancy or weight loss. Lifting the breast, elevating the nipple, and decreasing the size of the areola create this more youthful look. Breast reduction is a similar procedure except that the breast is also reduced in size. With both procedures, great care should be taken to sculpt the breast into the most aesthetically pleasing shape. Results are permanent but may be affected by significant weight gain, aging, or pregnancy.

Brow Lift

Also called a forehead lift, the procedure can eliminate lines and wrinkles in the brow - the same line that cause people to ask if you're tired or angry when you're not. Until recently, the only way to perform the surgery was to make a large incision at the top of the head, pull the skin back, and remove the excess skin. This left an unsightly scar that could be seen through thin hair or when hair was wet or blown by the wind. Today's endoscopic brow lifts result in a much more desirable outcome. Endoscopic brow lifts allow surgeons to make small incision in the hair line where a small camera is inserted which helps them look inside to raise the eyebrows. Once the brows are repositioned, sutures are placed on the inside, leaving minimal scarring. Results usually last five to ten years.

Recovery time is usually about two to four weeks, with final results apparent in up to two months.

Chemical Peel

Wrinkles and fine lines around the mouth and on the forehead and cheek areas can be improved with chemical peels, a non-invasive way to "turn back the clock."

With most types of peels, a chemical solution (such as alphy hydroxy acids, tricholoracetic acid, or phenol as a peeling agent) is applied to the entire face or to specific problem areas to peel away the skin's top layers, allowing for new cell growth that produces a smoother, tighter, younger-looking skin surface. Most patients prefer several light- to medium-depth peels over a one-time intense peel for reduced risk and shorter recovery times. Chemical peels can restore wrinkled, blemished, unevenly pigmented, or sun-damaged facial skin.

Recovery time is approximately two weeks, but the maturation of the new skin and lightening of the pinkness may take up to six months.

Facelifts

Reversing the aging process through facial rejuvenation is increasingly popular as more people want to look as young as they feel.

"To achieve the most natural results, a combination of procedures is often indicated so an individualized approach is best," says Dr. White. "Attention to detail is especially important in facial surgery due to the complicated anatomy. The goal of any facial rejuvenation procedure should be to create long-lasting, natural results as inconspicuously as possible. A bad face lift is obvious to all. A good facelift is known to one, but admired by many."

Facelifts can improve the most visible signs of aging by removing excess fat, tightening underlying muscles, and redraping the skin of the face and neck. While facelifts can be performed in various ways depending on the surgeon's preference and the patient's needs, in general, the skin is separated from the fat and muscle below. Fat may be trimmed or suctioned from around the neck and chin to improve the contour. The surgeon then tightens the underlying muscle and tissue, pulls the skin back, and removes the excess.

Results of a facelift typically last five to ten years. Recovery time for a facelift is usually one to three weeks, with final results apparent in about two months. Incision lines continue to refine for one year.

D Magazine mailed a letter to nearly 6,000 local doctors, encouraging them to visit www.dmagazine.com to cast their votes for the Best Doctors in Dallas. THE BENEFITS OF BREAST ENHANCEMENT

A complete tummy tuck involves making an incision near the pubic hairline, from hipbone to hipbone, yet the incision is "placed low so it is hidden by a bikini", says Dallas plastic surgeon Steven White, M.D. If necessary, sagging or separated vertical muscles in the abdomen are tightened with permanent, internal sutures. Abdominal fat is reduced using liposuction or excision and then the skin and tissue are pulled down so that excess skin can be removed. Liposuction in the upper abdomen can further improve shape overall. If needed, a new "hole" is created in the skin for the navel, then the skin and underlying tissue is stitched together at the incision. In many patients, depending on the amount of excess skin, a modified tummy tuck with a smaller incision and without an incision around the navel may be possible. Dr White adds, "Dramatic results are often seen with a tummy tuck since it narrows the waist, tightens the tummy and removes excess skin and fat."

Though rarely discussed, gynecomastia (male breast enlargement) affects 40 to 60 percent of men at some point in their lives, according to ASAPS. The condition may be due to increases in fat, actual breast tissue or a combination of both. A medical evaluation is imperative before proceeding with surgery.

"Advanced technology and techniques have significantly improved the treatment of gynecomastia. In the past, surgical treatment involved cutting out excess breast tissue, but today, this is often unnecessary," says Dallas plastic surgeon Steven J. White, MD. You can be assured that an ABPS-certified doctor has graduated from an accredited medial school, has completed at least a three-year residency in general surgery and at least a two-year residency in plastic surgery. Abdominoplasty. For some people, all the crunches in the world won't get rid of their belly. If you are one of these people, especially if you've been pregnant or lost a significant amount of weight, the solution may be a tummy tuck. This is major surgery that removes excess skin and fat from the middle and lower abdomen and tightens the stomach muscles to reduce a protruding abdomen. A new procedure called a belt lipectomy often works for people who have lost a lot of weight but still have loose skin. It tightens the skin around the abdomen and back. The mini-abdominoplasty is performed through a short C-section-type incision. This can be a good choice for women who have lost muscle and skin tone after having children.

Find out if the surgeon you are considering will perform the surgery in an accredited facility and employs a well-trained, specializes staff of nurses and anesthesiologists. Pay attention to the before and after photos of the surgeon's own patients to assess the quality of their work, and read the patient comments about the care they received.
  • Meet with the surgeon. The consultation should be a time for you to explain what you hope to achieve and get recommendations. This is also the time for you to interview the doctor. Does the surgeon take the time to really listen to you and answer all of your questions, or do you feel rushed? Does the surgeon explain things in a way that is easy to understand, or do you just get to talk to the nurse? Recognize that the amount of the time the doctor spends with you before surgery is often indicative of the time he will spend seeing you for follow-up visits. After surgery, do you see the doctor or just the nurse? How many times will you have to return as you heal? These are important questions to consider as you decide which doctor is right for you.
